Foveoschisis secondary to high myopia

Infrared and HD-OCT of the right eye in a 55-year-old African American female with high myopia (more than -6.00 D), BCVA: 20/25 OU Cartwheel appearance of the fovea in the infrared imaging is visible. HD- OCT demonstartes schisis in different layers of the retina (both NFL and OPL; notice stretching of the Muller cells); VMT is also present . Outer retinal layers are preserved which explains the good vision . She had the same findings in OS.

Massive SRH in Patient on Coumadin Being Treated for Exudative AMD

78-year-old white female using 1mg of warfarin for atrial fibrillation, who had a large PED, Type 1 lesion from AMD. Noticed acute darkening of vision one week after anti-VEGF injection. Has very large SRH, subRPE heme, and corrugated retinal appearance post RPE tear. Vision HM (from 20/100). 20/25 in the fellow eye that has dry AMD.

MEWDS

26-year-old female presented with unilateral vision loss. She c/o flashes and a peripheral scotoma. Vision was 20/100. On exam, she showed foveal granularity, mild disc edema, and white dots as seen. Three weeks later, white dots had resolved and vision improved to 20/25.

Astrocytic Hamartoma

Five year-old girl came for regular eye examination with no complaints. Medical and ophthalmic history was unremarkable. On examination, visual acuity was 20/25 in both eyes. A Retinal Astrocytic Hamartoma was seen adjacent to the right optic nerve (picture). The rest of the eye examination was normal and no Lisch nodules were seen. The patient was referred for a pediatric neurologist examination and MRI scan of the brain.

Congenital Retinal Macrovessel

41 y/o male who presented with acute onset of blurred vision OD. Visual acuity was 20/200 OD; 20/25 OS. Examination was consistent with congenital retinal macrovessel through the macula with intraretinal hemorrhage as seen in the fundus photo. Intravitreal bevacizumab was injected, and visual acuity improved to 20/40 at 4-week follow-up. MRA head and neck was ordered to rule out other vascular anomalies.

CSCR Mushroom Cloud

Late transit FA of a large active subfoveal CSCR leak. Focus is on peri-foveal vessels to give sense of height of large serous RD of macula. This patient presented with a BCVA of 20/200 and fluorescein and historic evidence of prior episodes of leakage. After discussion of known treatment options including observation, he was initially treated with rifampin and had partial resolution to 20/70 BCVA but this was short-lived with reaccumulation of the large serous detachment within 3 months. He then received sub-threshold micro-pulse laser photocoagulation with an 810 nm diode laser which resulted 1 month later in complete drying of the serous detachment and BCVA of 20/25.

Hypertensive Retinopathy

Patient came in complaining of spots in vision in both eyes. VA was 20/25 - right eye and 20/20- left eye. Fundus exam reveals little hemorrhages with cotton wool spots due to hypertension and anemia.

Patient with stable vision since her last appointment (20/30 right eye and 20/25 left eye) with worsening vitreomacular traction in the right eye. Patient is followed routinely for Plaquenil use.

Mild Patton's Lines in IIH - Initial Photos

18-year-old African American female with increased BMI with a history of headaches, nausea, transient diplopia and vision loss that she notices when getting up from her bed (and goes away after standing upright) for the last two weeks. Went to PCP and was treated for the flu, and after no improvement and visual symptoms known, was sent to ED. MRI did not show any masses and showed empty sella turcia. Vision 20/30 OD and 20/20 OS; no RAPD; IOP 15OU; no anterior segment or vitreous inflammation; discs are elevated with obscuration of the disc margins and some of the smaller vessels; there are no SVPs; there are mild Patton's lines temporally (see Initial Photos). The optic disc cube shows 360 degrees of RNFL thickening (see OCT). Was referred to near-ophthalmologist, Dr. Doyle. She obtained additional work-up, and LP opening pressure was high, and MRV showed bilateral transverse sinus stenosis. Patient showed steady improvement with medical therapy, that included weight loss and oral diamox. On her last visit with Dr. Doyle, vision has remained stable at 20/20-20/25 without an enlarged blindspot; there are SVPs and optic disc edema has resolved (see Post Treatment Photos); she is currently on 1000 mg of diamox and has lost 15 pounds, and no stinting procedure needed.

Ozurdex in AC

90-year-old patient with an Ozurdex implant that migrated into the AC and with the cornea decompensating. Patient recommended for urgent surgery to remove implant. Vision OD at this visit was CF @ 2ft, most recent visit vision is 20/400, PH 20/25.

Papilledema Probable to Pseudo Tumor Cerebri

A 24-year-old female presented severe headaches and vision loss over the past 6-months. VA was 20/40 with pinhole 20/25, right eye and 20/25, left eye. Fundiscopic exam reveals 3-4 plus papilledema of both optic nerves. Patient has VF defects inferior, nasal-right eye and enlarged blind spot with an inferior arcuate defect present. Patient will get MRI and work-up with treatment in the hospital.

Peripheral Drusen

Ultra-widefield fluorescein angiogram of an 83-year-old female with Peripheral Drusen affecting both eyes. The patient presented on 1/19/2022 with slightly decreased vision since her last appointment. Her vision was sc20/25-2 in the right eye. The physician did not believe that the peripheral drusen represents AMD and recommended monitoring. The patient also had mild diabetic retinopathy at the time of her visit.

Retinitis Pigmentosa

Bilateral fundus photography and fundus autofluorescence imaging of a 62-year-old male with Retinitis Pigmentosa. Patient reported visual field defects and dark adapting issues. Patient's vision at the time images were taken were sc20/20 of the right eye and sc20/25 of the left eye. Dr. Sutherland determined that based on the patient's lack of family history, the most likely route of inheritance is autosomal recessive.

Susac's Syndrome

Forty one year old white male who presented for acute painless visual loss OD. There was a previous similar transient history of visual loss involving the right eye associated with a possible ill-defined cerebrovasculitis, which improved spontaneously. There also was a history of auditory difficulties. The vision at presentation was 1/200 OD, and 20/20 OS. Baseline kodachromes and a fluorescein angiogram are presented. With treatment and over a four month timeframe, the visual acuity improved to 20/25+ OD, while the left eye remained stable at 20/20.
