  • Right Eye Color Photo With Hemorrhages in Case of CNVM With Angioid Streaks

    A 45 year old male came with chief complaint of blurring vision in right eyes since past 4 days. His vision is 6/12 in right eye and 6/9 in left eye. His vision was 14 mmHg in right eye and 16 mmHg in left eye. He was diagnosed with Angioid Streaks in both eyes about a year ago, then he developed choroidal neovascularization in his left eye 8 months ago, for which he received AntiVEGF injections x 3. Left eye is a stable eye now. Patient presented with right eye choroidal neovascularization in a case of Angioid Streaks on recent follow up. We have advised him right eye AntiVEGF injections x 3. In this image, the right eye color photo shows bleed from CNVM in case of angioid streaks.

  • Large Retinal Tear from a Shuttlecock Injury

    27 year old woman presenting with floaters and a shadow in her temporal visual field OS. Approximately one week earlier, she was injured in her left eye by a shuttlecock while playing badminton. Fundus exam reveals mild vitreous hemorrhage and a large retinal tear with a small cuff of surrounding SRF.

  • MIDD (Maternally Inherited Diabetes and Deafness) - Right AF

    Both right and left eyes have symmetrical ring of mottled hypo/hyper AF around the fovea and disc. The HyperAF areas correspond to RPE deposits on OCT as well as areas of blockage on FA, and drusenoid deposits seen on fundus photos. Disc drusen in right eye present as HyperAF spot 57 yo WF referred for AMD vs Pattern Dystrophy that was diagnosed 10 years ago. Reported some slow progressive vision loss in both eyes for distance and near. Denies nyctalopia or hemeralopia. Background medical history includes HTN, CVD, and DM. No family history of eye problems. Denied pentosan use. Anterior segment showed moderate cataracts (OD>OS). Posterior segment exam showed macular changes and mild NPDR. The macular appearance showed a symmetrical, paramacular ring of fleck-like drusenoid material with some faint focal areas of RPE hyperplasia. Fundus Photos, AF, OCT were performed as well as a gene test. Further questioning showed revealed that her mother and maternal grandmother had both diabetes mellitus and sensorineural hearing loss. The patient developed diabetes in her teens, and some high frequency hearing loss in her early twenties. She had not had a previous genetic test or diagnosis of MIDD. Gene testing is pending for the mitochondrial component. Invitae's retinal panel, which does not include mitochondrial disorders, only showed a variant of uncertain significance, HMCN1. I discussed this case with Dr. Freund, and it is similar to a the case report : Inoue M, Kiss S, Freund KB. MACULAR PIGMENT RINGS AS THE PRESENTING FINDING OF MITOCHONDRIAL MYOPATHY, ENCEPHALOPATHY, LACTIC ACIDOSIS, AND STROKELIKE EPISODES.   • Ruptured Retinal Artery Macroaneurysm

    This is a fundus photo depicting ruptured Retinal Artery Macroaneurysm (RAM) in the left eye of a 63 years old female. RAM is an acquired saccular or fusiform dilatation of the retinal arterioles that usually occur within the first three orders of bifurcation. The Superotemporal artery is the most common location. RAM may be asymptomatic or cause a number of complications such as macular edema, serous macular detachment, and hemorrhages.

    This is the Fundus Picture of a 56 years Female, who had come for Cataract Surgery of her left eye. Her best corrected vision in left eye was 6/12. She was diagnosed to have Congenital Retinal Macro vessel in her left eye on fundus exam. She underwent cataract surgery and vision improved to 6/9. She is kept under regular follow up.

  • Cilioretinal Artery Occlusion

    Color image capturing the left eye of a 32-year-old female. Despite a negative ophthalmological and medical history, she reported three days of blurred vision and a paracentral scotoma in her left eye, while maintaining central vision. The image reveals retinal whitening, extends from the parafoveal region to the inferotemporal arcade indicative of cilioretinal artery occlusion. Following this observation, the patient was referred for systemic assessment to explore the underlying etiology of the occlusion.

    This is Fundus Photograph of left eye of 53 year male depicting an area of Retinal Necrosis with few Retinal Haemorrhages suggestive of CMV Retinitis. Areas of Perivascular Exudation also seen. On investigations, the patient was found to be HIV positive. He was started on Anti Retro Viral treatment after physician opinion.

  • Retinal Fold

    A 38-year-old man underwent vitrectomy in the left eye due to a giant tear in the upper retina. SF6 gas was used as endotamponade. During the post-surgical check-up, it was identified that the patient developed a full-thickness retinal fold due to retinal slippage during fluid-air exchange. As the fold was away from the macular area, it was decided to observe the patient. Three weeks after the surgery, his best-corrected visual acuity was 20/30.

    Ultrawide fundus photograph of the left eye demonstrating extensive peripheral hemorrhagic exudative detachment in a 79 yo Caucasian female with prior history of non-exudative AMD. Recent diagnosis of Acute myeloid leukemia with low platelet count which might have contributed to the above presentatuon. Please note the temporal subretinal hemorrhage as well as RPE atrophy and hyperplasia in the macula.

Notice multiple coin shaped hypoautofluorescent pacthes within central 20 degrees which are coalescing centrally.     Ultra-widefield fundus photograph of an 81 year old female with a Methotrexate bubble following an Intravitreal Injection for Proliferative Vitreoretinopathy. Patient has been presenting to the office for two week interval Methotrexate injections in her left eye. The image was taken prior to her eighth injection which revealed a residual Methotrexate bubble in her inferior retinal image. This patient was seeing "lots" of floaters, as well as having visual acuity of cc20/400 cc20/200 PH.

    Ultra-wide field fundus fluorescein angiography (FFA) of the left eye from an asymptomatic, healthy 33-year-old woman who was referred to the retina clinic from a refractive surgery unit due to the presence of vascular anomalies and hard exudates in both eyes. FFA revealed the characteristic sacular aneurysms at the bifurcation of retinal arterioles in the posterior pole, together with microvascular anomalies and capillary closure peripherally.

  • Thioridazine-toxicity

    61 yo male with PMH of longstanding schizophrenia since 20s with secondary intellectual disability presented with decreased vision following a recent stroke. He was found to have bilateral chorio-retinal atrophy involving posterior pole with scalloped edges and coin shaped atrophic area at margins extending into mid-periphery, diagnosis most concerning for intermediate stage thioridazine toxicity given the history. Mother could find handwritten prescriptions from 1990s when he was on Thioridazine 800 mg daily for unknown period of time. Patient had better vision in the left eye which was affected by recent stroke and prompted him to seek medical care. Fundus photograph of the right eye is demonstrated here.

    Left ultra-wide field photograph of an 81-year old female with a choroidal detachment affecting her left eye. Patient had a stent placed November, 2021 and following the procedure she complains of variable blurred vision and severe constricted visual fields. She presented at our office with flashes a month prior but without pain or floaters.
