- Initial OCT B-scan demonstrating a large hyperreflective intraretinal hemorrhage with marked posterior shadowing, causing distortion and partial obscuration of the underlying macular architecture. Adjacent intraretinal hyperreflective deposits/exudation and retinal thickening are present. Hyperreflective dots are also visible in the vitreous, consistent with associated subhyaloid/premacular hemorrhage. Findings are consistent with acute hemorrhagic and exudative changes secondary to a ruptured retinal arterial macroaneurysm.
