  • Chronic Total Retinal Detachment

    Ultra-wide field pseudocolor image of a 57-year-old male with a chronic total retinal detachment affecting his right eye. Patient presented with klebsiella endophthalmitis in UK, and was in medically induced coma with tracheostomy. He awoke after sedation with loss of vision in both eyes, later developing a retinal detachment in both eyes. He had his first repair with gas then with silicone oil. Although he is developing band K and corneal decompensation due to oil in the AC, he has a chronic cicatricial retinal detachment with subretinal and preretinal PVR with large stretch breaks. His vision may worsen with surgery and his eye is hypotenous and if repair is possible, oil will need to be replaced and thus may still migrate to the AC thus would defer intervention until absolutely necessary given risks of vision loss in his only seeing eye. Given this is his only eye, observation is recommended at this time.

    A middle-aged man underwent complex retinal detachment repair with vitrectomy, membrane peeling, relaxing retinectomy, and silicone oil tamponade. This is a wide-field image immediately after methotrexate injection during a postoperative clinic visit, for the prevention of proliferative vitreoretinopathy. The methotrexate bubble is seen inferiorly.

    Ultra-wide field pseudocolor fundus photograph of a 64-year-old female with a treated retinal detachment with proliferative vitreoretinopathy. Patient has a history of complex retinal detachments that have been treated multiple times. On exam 8-22-19, there were large macular holes with LP vision. There was a long discussion about guarded nature of her condition and goals or trial for repair including globe sparing prevention of phthisis.

  • Proliferative vitreoretinopathy in complex retinal redetachment

    A 58-year-old female underwent phacovitrectomy + scleral buckling by retinal detachment. Silicon oil was used as endo tamponade. In the post-surgical period, the formation of a subretinal PRV is observed with a significant circumferential contraction that generates an inferior redetachment. It was decided to undergo a vitreous cavity lavage to remove membranes + 180° retinectomy.

    Postoperative fundus photo of the margin of the inferior nasal retinectomy site. The retinectomy was used intra-operatively to flap the retina over and remove the subretinal bands. The scleral buckle effect and acute white endolaser marks are present at the edge of the retinectomy.

    56 year-old male with a history of retinal detachment OD, complicated by recurrent RD with PVR and cataract. He underwent multiple surgical procedures that included scleral buckling using a 3mm silicone sponge, membrane peel, inferior retinectomy, endolaser, lensectomy, and secondary IOL placement. Current VA is 20/60 OD.
