  • 360 Retinotomy in a closed Funnel combined Tractional and rhegmatogenous retinal detachment

    This is the results at 6 months of a Bimanual 23 G-PPV with a very extensive and posterior 360 retinotomy for the management of a combined longstanding closed funnel RD, with submacular membranes, intraretinal PVR. Preop VA was a doubtful light perception. Borders of the retinotomy are stable at 6 months under 1300 Cs Silicon oil with some pigmented PVR developping the edges. Macula appears spared. Silicon oil emulsification droplets are well visualized beneath the superior temporal arcade.

    57-year-old male patient who underwent phacoemulsification. Pre-op vision was 20/70. Complicated surgery. With vitreous loSS, cataract surgeon decided to place the foldable intraocular lens in the AC. Presented in the clinic with a vision of hand movement, with intra-ocular preSSure of 65 mmHg. He was managed by the same surgeon who gave Cosopt TID, and Alphagan QID.

    Peripheral endoscopic imaging of the pars plana region. The second instrument can be seen in the sclerotomy site moving in and out. Scanning of the more anterior region reveals a large particle of retained lens material following phacoemulsification. This particle was responsible for recurrent inflammation and cystoid macular edema. Subsequent removal ended the inflammation and retinal edema.

    Ultrasound biomicroscopy of a post-operative eye (status post trabeculectomy and phacoemulsification) reveals a patent ostium on the right side, along with an intraocular lens in position. A hyphema is observed displaying small convection currents, creating a circular motion pattern due to the temperature gradient between the iris and cornea. Notably, the blood flow can be seen circulating toward the trabeculectomy site.

  • Inflammatory pupillary membrane in patient with endophthalmitis

    Anterior segment photography of a 54-year-old woman with post phacoemulsification endophthalmitis. She did not improve after first intravitreal antibiotics injection and develop an inflammatory pupillary membrane. After two vitrectomies, and a complete three intravitreal injections scheme, we decided to remove the intraocular lens and capsules.

    B-mode transverse ultrasound scan of an eye with recent posterior capsule rupture during phacoemulsification shows hyperechoic punctate echoes in the vitreous (consistent with residual viscoelastic material) along with lens fragments in the subhyaloid space.

  • Retinal Detachment Repair With Silicone Oil and Scleral Buckle, Fourteen Years Later, With Visual Acuity of 20/25

    65-year-old woman s/p scleral buckle 14 years ago. Two weeks later, the retina re-detached, and vitrectomy, laser, and silicone oil procedure was performed. Patient remains 20/25 with correction fourteen years later. The cornea is clear, there is no oil emulsification, and there is a stable, moderately inferiorly subluxated PCIOL (as it was prior to RD surgery). IOP is 17 on Cosopt BID.
