  • Quesadilla-Like Folding of the Retinal Pigment Epithelium After a Large RPE Tear in Neovascular AMD

    Case description A 63-year-old woman with neovascular age-related macular degeneration (nAMD) in the right eye underwent a loading course of three intravitreal aflibercept injections. Following treatment, she developed a large retinal pigment epithelium (RPE) tear involving nearly the entire macular area, with marked retraction and folding of the detached RPE. Color fundus photograph Color fundus photography of the right eye shows a broad area of macular RPE disruption and retraction. The torn RPE is displaced and folded upon itself within the central macula, producing a well-demarcated elevated pigmented lesion surrounded by an area of relative RPE absence.

    Case description A 63-year-old woman with neovascular age-related macular degeneration (nAMD) in the right eye underwent a loading course of three intravitreal aflibercept injections. Following treatment, she developed a large retinal pigment epithelium (RPE) tear involving nearly the entire macular area, with marked retraction and folding of the detached RPE. Fundus autofluorescence Fundus autofluorescence demonstrates a large area of marked hypoautofluorescence corresponding to loss of the RPE and exposure of the underlying tissue. In contrast, the retracted and folded RPE appears markedly hyperautofluorescent, sharply delineating the displaced pigment epithelium and emphasizing the extensive configuration of the tear.

Spectral-domain OCT Spectral-domain optical coherence tomography demonstrates abrupt interruption of the RPE layer with extensive retraction of the torn RPE. The displaced RPE forms a prominent, sharply elevated hyperreflective fold with dense posterior shadowing. Adjacent areas lacking RPE show increased optical transmission into the choroid. The folded tissue creates a striking double-layered configuration, resembling a folded “quesadilla,” corresponding to the markedly retracted edges of the large RPE tear.
