Fundus camera VISUCAM 524(Carl Zeiss Meditec AG, Jena, Germany) - Description
- Case description A 63-year-old woman with neovascular age-related macular degeneration (nAMD) in the right eye underwent a loading course of three intravitreal aflibercept injections. Following treatment, she developed a large retinal pigment epithelium (RPE) tear involving nearly the entire macular area, with marked retraction and folding of the detached RPE. Spectral-domain OCT Spectral-domain optical coherence tomography demonstrates abrupt interruption of the RPE layer with extensive retraction of the torn RPE. The displaced RPE forms a prominent, sharply elevated hyperreflective fold with dense posterior shadowing. Adjacent areas lacking RPE show increased optical transmission into the choroid. The folded tissue creates a striking double-layered configuration, resembling a folded “quesadilla,” corresponding to the markedly retracted edges of the large RPE tear.
