13 year old male presented with new blind spot 6 weeks s/p RD repair with cryo/scleral buckle/prophylaxis laser with gas bubble. New RD involving the macula, posterior to scleral buckle, secondary to PVD. Small gas bubble remaining. Pt was brought back to OR for repeat PPV and silicone oil repair

Retinal Autograft Postoperative Day 1

20-year-old boy presented with recurrent retinal detachment post silicone oil removal with a macular hole. During surgery a retinal graft was created from the detached retina and positioned on the macular hole. silicone oil was injected at the end of surgery. One can see the diathermy marks on all sides of the edges of the graft.

Submacular PFO

This is a 53-year-old gentleman that was referred to us for a second opinion with an inoperable RD with PVR after 3 failed attempts. We performed a PPV, membranectomy, scleral buckling procedure, with silicone oil injection. This case did not require PFO. You can imagine our surprise when we discovered submacular PFO postoperatively. It is very difficult to see the PFO on the Optos. The infrared shows it clearly, with confirmation of the submacular space on the SD-OCT.

Retinal Detachment with Giant Retinal Tear and Macular Hole

A 61-year-old-male patient presented with sudden diminution of vision in the right eye due to retinal detachment with giant retinal tear and macular hole. Best corrected visual acuity (BCVA) at presentation was 20/200. A 23 G vitrectomy was performed. The edges of the tear were unrolled and complete retinal re-attachment under PFCL was achieved. A 360 degree intraoperative endolaser was performed on the peripheral retina as well as around the edges of the tears. PFCL was exchanged with silicone oil 5000cs as final tamponade. At six-months follow-up retina was attached and macular hole was repaired. Best-corrected visual acuity is 20/125 at present.

Silicone Oil Droplets in the Vitreous

A 65-year-old female presented with an acute decrease in VA in the left eye 48 hours after an intravitreal injection of an antiangiogenic drug. Slit-lamp examination showed AC cells (4+) and vitritis. No pain or hyperemia. Multiple silicone oil droplets were seen in the anterior vitreous.

Ultra-wide field pseudocolor fundus photograph of a 64-year-old female with a treated retinal detachment with proliferative vitreoretinopathy. Patient has a history of complex retinal detachments that have been treated multiple times. On exam 8-22-19, there were large macular holes with LP vision. There was a long discussion about guarded nature of her condition and goals or trial for repair including globe sparing prevention of phthisis.

Retinal Detachment Repair With Silicone Oil and Scleral Buckle, Fourteen Years Later, With Visual Acuity of 20/25

65-year-old woman s/p scleral buckle 14 years ago. Two weeks later, the retina re-detached, and vitrectomy, laser, and silicone oil procedure was performed. Patient remains 20/25 with correction fourteen years later. The cornea is clear, there is no oil emulsification, and there is a stable, moderately inferiorly subluxated PCIOL (as it was prior to RD surgery). IOP is 17 on Cosopt BID.

Silicone Oil Large and Small Droplets 8 Days Post Avastin (Avella) Injection

The 88-year-old man had an Avastin injection (compounded by Avella) OD for AMD 8 days prior to this photo. He immediately complained of floaters after the injection which persisted 8 days later. The Optos photo, taken 8 days after the injection, shows a large silicone oil bubble about 1DD in size above the ST arcade and more than a dozen smaller 0.1DD bubbles over the macula suspended in the vitreous gel.
