    This patient was sent for a possible retinal detachment. Extreme difficult view of posterior pole due to asteroid hyalosis. After B-Scan was performed it was determined patient did not have a retinal detachment, only posterior vitreous detachment.

    In asteroid hyalosis, accumulations of calcium soaps dispersed throughout the vitreous produce bright echoes in the usually echolucent vitreous. The appearance of asteroid hyalosis should not be confused with that of vitreous hemorrhage or vitritis. Many of the larger aggregates in asteroid hyalosis are easily seen as the gain is reduced to below 60 db, unlike vitreous hemorrhage or vitritis which usually disappears at low gain settings. There is also an area of clear echolucent vitreous between the posterior hyaloid face and the asteroid particles, which is usually not present in vitreous hemorrhage or vitritis.

    VA 20/20. In asteroid hyalosis, there is a space between the posterior hyaloid face and the calcified asteriod lesions. Use gain to differentiate between VH and asteroid hyalosis (asteroid still present when gain is decreased).

    Ultrasound B-Scan shows discrete mobile point / dot like bright echoes in the vitreous cavity with an acoustic clear zone between the dot echoes and the posterior globe wall with no posterior acoustic shadowing. The corresponding A scan shows high spikes. OCT macula confirms the same.

  • Posterior Subcapsular Changes

    Sixty seven year old black female with a history of progressive love of vision over the past several years. Several years ago her visual acuity was 20/40 in each eye. This gradually became reduced to 20/50 in the right eye and 20/70 in the left eye. She had increased posterior subcapsular changes in the left eye, and had bilateral intraocular lens surgery. Intraocular pressures were within normal limits and the patient exhibited bilateral asteroid hyalosis. The patient's bilateral pigmentary changes appeared to have increased since she was last seen. Also there has been no evidence of any diabetic retinopathy.
