Retinal Detachment with PVR

Optomap RGB of an 48-year-old man with a retinal detachment with PVR. Patient is 6 weeks s/p RD repair with giant HSRT. Patient has new PVR noted on post op exam causing the retina to re-detach. Patient is having to have a 2nd surgery to remove the scar tissue and have silicone oil placement. Will continue close follow up care.

Pneumatic Retinopexy

Fundus photograph of a pneumatic retinopexy. The upper photo taken just 30 minutes after C3F8 gas injection shows rhegmatogenous retinal detachment with superior temporal horseshoe tear and gas bubbles resembling fish-eggs. After two days with appropriated head position (botton photo), the retina is attached and laser photocoagulation was performed on the border of the break. A Single great gas bubble was formed.

Intralenticular Gas

Ultrasound biomicroscopy image of resolving intralenticular gas at office visit approximately 4 weeks later (B). The lens remained unchanged in thickness at 3.78 mm before and after resolution of the gas bubble, but developed a cataract.

Subretinal Gas After Pneumatic Retinopexy

Pseudocolor fundus photograph of a 68-year-old man who presented with a macula-on rhegmatogenous retinal detachment with a single horseshoe tear at 12 o'clock. Pneumatic retinopexy was performed with cryopexy to the retinal break. He returned to clinic three days later and the entire SF6 gas bubble was noted to have migrated to the subretinal space through the retinal break. Pars plana vitrectomy was performed that day with retinal reattachment and improvement in vision to 20/40 now 6 months postoperatively.

13 year old male presented with new blind spot 6 weeks s/p RD repair with cryo/scleral buckle/prophylaxis laser with gas bubble. New RD involving the macula, posterior to scleral buckle, secondary to PVD. Small gas bubble remaining. Pt was brought back to OR for repeat PPV and silicone oil repair

Subretinal Migration of SF6 gas: Postop day 1

67-year-old with a new Rhegmatogenous Retinal Detachment, macula on, with posterior retinal tear, superotemporally. Patient underwent 25 G vitrectomy, endo laser and SF6 gas injection and was advised prone position postoperatively. The image shows the appearance of the fundus on postoperative day 1 through the gas bubble . Patient was advised sitting position subsequently. Resolved completely over the next five days.

Submacular Hemorrhage after C3F8 Pneumatic Displacement

The patient underwent urgent intravitreal injection of recombinant tissue plasminogen activator and pneumatic displacement with 100% C3F8 gas. The following day, the sub-macular haemorrhage was largely displaced away from the fovea inferiorly and nasally, with the gas bubble seen superiorly.
