    Description
    This anterior segment photograph shows an aphakic eye in a patient with a remote history of traumatic cataract in childhood, who subsequently underwent cataract extraction with superior iridectomy, resulting in surgical aphakia. The image demonstrates residual capsular/cortical lens material with marked chronic capsular phimosis and extensive iridocapsular synechiae, leading to pupillary seclusion. These chronic structural changes have resulted in the formation of a new functional central opening, clinically resembling a secondary neopupil. This case illustrates how long-standing postoperative and post-traumatic anterior segment changes can significantly alter pupillary anatomy and create an alternative pupillary aperture.
