Anemic Retinopathy Related Retinal Hemorrhages

Anemic retinopathy related retinal hemorrhages in a 24 years old male with Hb of 4.2gm/ dl. The manifestations of anemic retinopathy are nonspecific and may closely simulate hypertensive or diabetic retina. Retinal changes in anemia are cotton wool spots, venous tortuosity, and hemorrhages which may be present at all levels of the retina and choroid. All retinal hemorrhages can occur when Hb falls below 8 g/100 ml or if the platelet count falls below 50,000/cumm. The combination of severe anemia and thrombocytopenia is likely to produce retinal hemorrhages. The Roth’s spots or white centre hemorrhages are typically associated with bacterial endocarditis , anemia and other systemic conditions. The white center is suspected to represents focal ischemia, inflammatory or infectious infiltrate, fibrin or accumulation of neoplasticism cells.

Roth Spots - Bacterial Endocarditis

Initial presentation; 29-year-old white female denied ivdu p/c with acute scotoma due to the sub-ILM foveal heme. She did have some roth spots in both eyes. There was a focal area of periphlebitis just superior to the fovea OD. Work up for roth spots and retinal vasculitis initiated. She did have a low grade fever that she attributed to a urinary tract infection being treated by her PCP.

Few weeks later; some areas resolved, and new roth spots; sub-ILM foveal heme in right eye resolved and vision back to baseline; full work-up pending.

Two months later; interim was hospitalized for bacterial infection of mitral valve; roth spots resolved.

Septic emboli. The fundus photographs illustrate a man with subacute bacterial endocarditis who presented with branch retinal artery occlusion in the left eye and a single Roth spot in the right eye. 29 yo wf denied ivdu p/c with acute scotoma. OCT shows sub-ILM heme in foveal region (left) 20/300 that resolved spontaneously a few weeks later, back to baseline acuity (right).
