Extensive Traumatic Choroidal Rupture With Associated Full-Thickness Macular Hole

A patient presented after a motorcycle accident resulting in severe blunt trauma to the right eye. Color fundus photography demonstrates an extensive curvilinear choroidal rupture extending from the peripapillary region toward the superior temporal vascular arcade and superonasal macula. The rupture is characterized by marked disruption of the retinal pigment epithelium–Bruch membrane–choroid complex, with direct visualization of the underlying choroidal vasculature. A well-defined, round traumatic full-thickness macular hole is also present.

A patient presented after a motorcycle accident resulting in severe blunt trauma to the right eye. Color fundus photography demonstrates an extensive curvilinear choroidal rupture extending from the peripapillary region toward the superior temporal vascular arcade and superonasal macula. The rupture is characterized by marked disruption of the retinal pigment epithelium–Bruch membrane–choroid complex, with direct visualization of the underlying choroidal vasculature. A well-defined, round traumatic full-thickness macular hole is also present. Fundus autofluorescence reveals a broad hypoautofluorescent curvilinear band corresponding to the choroidal rupture, consistent with extensive retinal pigment epithelium loss or disruption. Focal areas of hyperautofluorescence along the margins of the rupture likely represent stressed or altered adjacent retinal pigment epithelium. The traumatic macular hole is also clearly delineated by an abnormal foveal autofluorescence pattern. The multimodal imaging findings are consistent with extensive traumatic choroidal rupture involving the posterior pole with associated traumatic full-thickness macular hole following blunt ocular trauma.
