A 13 year old female presented to the OPD with complaints of diminution of vision in both eyes since 3 months. She was not evaluated by any ophthalmologist before. On examination, bilateral disc edema was noted and she was advised for MRI brain and orbits which revealed a fourth ventricular ependymoma.

MRI Brain - Eyes on the Brain: Bilateral Papilledema Unmasking a Fourth Ventricular Ependymoma

Sagittal T1-weighted magnetic resonance imaging of the brain demonstrating a large heterogeneous posterior fossa mass centered within the fourth ventricle. The lesion compresses the brainstem and obstructs cerebrospinal fluid pathways, resulting in hydrocephalus.
