  • Berlin Edema with Peripheral Commotio and Choroidal Rupture After Blunt Trauma

    A 13 year-old patient presented 2 days after blunt trauma to the right eye (OD) caused by a slingshot stick, with immediate pain and ocular bleeding. Best-corrected visual acuity was counting fingers. Color fundus photograph of the right eye (OD) obtained with Zeiss Clarus shows clear media and a well-perfused optic disc with normal coloration and minimal blurring of its margins. Diffuse peripapillary retinal whitening with subtle elevation is present, extending 360 degrees around the optic nerve head. The macula is involved, demonstrating whitish discoloration and mild elevation extending from the nasal side. Retinal vessels are mildly tortuous. Inferiorly, a choroidal rupture measuring approximately 10–12 disc diameters is noted, with a pinkish-white appearance, hyperpigmented borders, and visible underlying choroidal vessels. Inferonasal to the optic nerve head, a small epiretinal hemorrhage with a whitish center is present. The peripheral retina demonstrates circumferential whitening consistent with commotio retinae.

    A 13 year-old patient presented 2 days after blunt trauma to the right eye (OD) caused by a slingshot stick, with immediate pain and ocular bleeding. Macular OCT of the right eye (OD) demonstrates multiple hyperreflective dots within the vitreous cavity. The vitreoretinal interface is preserved. There is loss of the normal foveal depression due to the presence of subretinal fluid involving the macula. The inner retinal layers maintain normal architecture, with preserved continuity and parallelism. The outer retinal layers, particularly on the nasal side, show focal thickening and irregularity. These findings correlate with the areas of retinal whitening observed clinically and are consistent with traumatic outer retinal disruption in the setting of Berlin edema.

Anterior segment photographs demonstrate iris and lens coloboma with superior lens subluxation. Color fundus imaging reveals an inferior chorioretinal coloboma sparing the macula. Macular OCT demonstrates preserved foveal anatomy, whereas OCT through the coloboma illustrates the structural transition at the colobomatous margin.
