A 70 year-old female underwent surgery for a small full-thickness macular hole in the right eye. She was treated with pars plana vitrectomy, transscleral cryotherapy, endolaser photocoagulation, fluid–air exchange, and intravitreal SF6 gas injection. On postoperative day 1, ultra-widefield imaging revealed a well-attached retina with approximately 90–100% SF6 gas fill. The 360-degree endolaser photocoagulation marks were seen arranged circumferentially, resembling a “garland,” giving an artistic appearance to the therapeutic laser pattern.

A 44 year-old male presents with a complex ocular anomaly. Left eye examination reveals iris and extensive retinochoroidal coloboma involving both the optic disc and macula, associated with a dislocated crystalline lens. The large excavated colobomatous defect with disc–macula involvement suggests a guarded visual prognosis, further compounded by lens displacement. The fellow (right) eye is aphakic following prior vitreoretinal surgery for a dislocated lens, with barrage laser marks surrounding a coloboma that involves the disc but spares the macula. This case highlights the coexistence of posterior segment coloboma with lens subluxation/dislocation, posing both anatomical and therapeutic challenges.

Swept-source ultra-widefield OCT angiography (SS-UWF OCTA) of the left eye of a 74 year-old woman with chronic ischemic central retinal vein occlusion (CRVO) demonstrates extensive capillary non-perfusion along with macular ischemia. The preserved major retinal vessels radiating from the optic disc contrast sharply with the surrounding avascular retina, creating the appearance of a solitary vascular tree standing within a vast ischemic landscape. This image highlights the ability of SS-UWF OCTA to delineate the full extent of retinal ischemia in chronic CRVO without the need for dye-based angiography.
