Iris transillumination in a patient with herpetic iritis
Apr 11 2022 by Aniruddha K Agarwal, MD
The patient is a 45 year old male with who presented with uveitis poorly responsive to topical steroids. Iris transillumination and elevated IOP on transillumination suggested herpetic etiology. Inflammation responded to the addition of oral antivirals.

Acute syphilitic posterior placoid chorioretinitis
Apr 24 2022 by Aniruddha K Agarwal, MD
Green-light fundus autofluorescence (FAF) of the right eye from a 55-year-old man with risk factors for sexually trasnmitted diseases who presented to the retina clinic for a central scotoma. Funduscopy revealed a placoid lesion in the posterior pole. FAF highlights a hyperautofluorescent placoid lesion involving the macula with granular hyperfluorescence. The patient tested positive for syphilis and received intravenous penicillin treatment.

Vogt-Koyanagi-Harada Disease
Apr 24 2022 by Aniruddha K Agarwal, MD
A 38-year-old woman of Asian descent with no ophthalmological or systemic history presented to the emergency eye clinic with a 1-week complaint of headache and bilateral vision loss. Funduscopy revealed bilateral serous neurosensory detachments. The presence of lymphocytosis in cerebrospinal fluid and mild acute sensorineural hearing loss confirmed the diagnosis of uveomeningoencephalitic syndrome (Vogt-Koyanagi-Harada disease).

Idiopathic retinal vasculitis, aneurysms and neuroretinitis
Apr 24 2022 by Aniruddha K Agarwal, MD
Ultra-wide field fundus fluorescein angiography (FFA) of the left eye from an asymptomatic, healthy 33-year-old woman who was referred to the retina clinic from a refractive surgery unit due to the presence of vascular anomalies and hard exudates in both eyes. FFA revealed the characteristic sacular aneurysms at the bifurcation of retinal arterioles in the posterior pole, together with microvascular anomalies and capillary closure peripherally.
