Thomas A. Ciulla, MD, MBA, FASRS » Polypoidal Choroidal Vasculopathy

An 86 year old woman was referred with blurred central vision OD and submacular hemorrhage. Visual acuity measured 20/40. Macular exam revealed a PED versus a polypoidal lesion temporal to the disk surrounded by subretinal hemorrhage and subretinal fluid. Angiography revealed peripapillary hyperfluorescence with mild late leakage, as well as blockage by the subretinal hemorrhage noted clinically. OCT revealed dome like elevation nasal to the foveal with surrounding subretinal fluid.ICG angiography, not shown here, is better than fluorescein angiography in differentiating the lesions.
