- OCT Showing a lamellar macular hole with a tangential epiretinal membrane and classic "moustache sign" foveoschisis — oblique hyperreflective tracts radiating from the hole edges from ERM traction. The outer retinal layers (ellipsoid zone, RPE) remain intact at the base, a favorable prognostic sign that also informs the surgical approach (combined ERM/ILM peeling).
