    Description
    A 60 year old white female was found unconscious in her home. She was rushed to the hospital where a CAT scan of her head revealed a large sub-arachnoid hemorrhage. A carotid angiogram showed a ruptured aneurysm of the posterior communicating artery on the right side. The next day, she had a caniotomy to evacuate the intracranial hemorrhage. On examination, visual acuity was 20/100 OD and 20/80 OS. The right pupil was approximately 8mm in dim light and reacted to approximately 6mm with light. The left pupil was approximately 7mm in dim light and reacted to approximately 2.5mm with light. There was no afferent pupillary defect. There was a large angle exotropia with only abduction of the right eye. Ductions in the left eye were full. External examination revealed a 3mm ptosis on the right. Slit lamp examination was within normal limits. Tensions by applanation were 19 OD and 16 OS.
