Presumed Ocular Histoplasmosis Syndrome (POHS) with Choroidal Neovascular Membrane (CNVM) in the Right Eye: Treatment Response After One Dose of Anti-VEGF Therapy (Bevacizumab)
Oct 20 2026 by Fernando Pellerano, MD
A 48-year-old man presented with acute vision loss OD. BCVA was 20/80 OD and 20/20 OS. (A) Fundus examination of the right eye showed mild peripapillary atrophy (PPA), peripheral chorioretinal scars, and macular chorioretinal scarring with hemorrhage, concerning for CNVM. (B) Fundus autofluorescence (FAF) of the right eye showed hypoautofluorescence in areas of chorioretinal scarring. (C) Optical coherence tomography (OCT) of the right eye showed cystoid macular edema, subretinal hyperreflective material, and subretinal fluid. (D) OCT one month after a single dose of bevacizumab showed resolution of intraretinal fluid (IRF) and subretinal fluid (SRF), and visual acuity improved to 20/25. The patient was treated and extended with bevacizumab and faricimab-svoa, receiving a total of six intravitreal injections OD. Two years after discontinuing treatment, he remains free of active CNVM with preserved vision. The left eye showed histo spots and PPA without CNVM (not shown).
