    28 year-old patient with a history of right-eye leukocoria since the age of 15 and secondary sensory exotropia with ocular toxocariasis. Color fundus montage of posterior pole of the right eye showing a large elevated whitish juxtapapillary fibrogranulomatous lesion consistent with ocular toxocariasis. The lesion is connected to the optic disc by a dense fibrovascular band and is associated with marked vitreoretinal traction, radial retinal folds, vascular displacement, and distortion of the posterior pole. Surrounding areas of chorioretinal atrophy and pigmentary change are also present, predominantly temporal to the lesion.

    28 year-old patient with a history of right-eye leukocoria since the age of 15 and secondary sensory exotropia with ocular toxocariasis. The fundus photograph showed an elevated, whitish fibrogranulomatous lesion in the posterior pole, adjacent to the optic disc, associated with vitreoretinal traction bands, converging retinal folds, and marked distortion of the posterior pole architecture, with dragging of the retinal vasculature and tractional deformation of the optic disc.

  • Fundus Autofluorescence in Ocular Toxocariasis With a Posterior Granuloma

    28 year-old patient with a history of right-eye leukocoria since the age of 15 and secondary sensory exotropia with ocular toxocariasis. Fundus autofluorescence showed an intensely hyperautofluorescent central lesion with relatively well-defined borders, corresponding to the fibroinflammatory granuloma. A heterogeneous autofluorescence pattern surrounded the lesion, with irregular marginal hypoautofluorescent areas, likely related to fibrosis, retinal pigment epithelium alterations, and a blocking effect. Mottled hyper- and hypoautofluorescent changes were also identified in the surrounding retina, consistent with chronic retinal pigment epithelium involvement. The optic disc showed homogeneous autofluorescence, and the tractional bands were associated with distortion of the posterior pole architecture.

  • Early-phase Fluorescein Angiography in Ocular Toxocariasis With a Posterior Granuloma

    A 28 year-old patient with a history of right-eye leukocoria since the age of 15 with fibrogranulomatous lesion consistent with ocular toxocariasis. Early phase : Fluorescein angiography showed progressive filling of the retinal vasculature, with extensive hypofluorescence corresponding to the central mass, primarily due to blockage by the elevated fibrogranulomatous tissue. The retinal vessels were distorted and displaced around the lesion, with several appearing to be drawn toward or folded into it. Early hyperfluorescence was present along the granuloma margins, in some peripheral chorioretinal areas, and at the optic disc.

    A 28 year-old patient with a history of right-eye leukocoria since the age of 15 with fibrogranulomatous lesion consistent with ocular toxocariasis. Intermediate phase: There was a progressive increase in hyperfluorescence along the borders and surface of the lesion, with staining of the fibrovascular bands and tractional areas. Irregular perilesional hyperfluorescent areas became more evident, consistent with staining of inflammatory or scar tissue, while a central hypofluorescent zone persisted because of blockage. Optic disc hyperfluorescence also increased, suggesting disc staining or leakage, with possible focal vascular leakage in areas of retinal traction.

  • Late-phase Fluorescein Angiography in Ocular Toxocariasis With a Posterior Granuloma

    A 28 year-old patient with a history of right-eye leukocoria since the age of 15 with fibrogranulomatous lesion consistent with ocular toxocariasis. Late phase: Marked central and perilesional hyperfluorescence increased in both intensity and extent, with diffuse fluorescein accumulation within and over the lesion, consistent with late staining and active leakage from the inflammatory granuloma. Central and peripheral areas of blockage persisted, accompanied by late optic disc leakage and staining of the chorioretinal scars and lesion margins.
