Visual Acuity: OD 20/200, OS Counting fingers

KODACHROME PICTURES:
Color views of the right posterior pole reveal a ray of subretinal fibrosis surrounding the right optic disc with choroidal neovascularization extending out and involving the right macula region. Color views of the left posterior pole also reveal some fibrotic change at the temporal edge of the left disc and with a fibrotic lesion extending out and involving the left macula region with sub-retinal blood present.

FLUORESCEIN ANGIOGRAM:
Fluorescein dye transit through the left eye reveals areas of hypo and hyperfluorescence representing choroidal neovascularization involving the left macular region with subretinal blood. Fluorescein dye transit through the right eye also reveals hyperfluorescence around the disc and extending out involving the right macula.

IMPRESSION:
The patient has bilateral peri-papillary sub-retinal fibrosis with macula involvement with choroidal neovascularization.
