This 84 year old white male was originally seen in November, 1992, complaining of inability to read small print. He had been examined previously by an ophthalmologist who recommended cataract surgery. Currently, he is taking Nitroglycerin and Cardizem.

Best corrected vision was 20/60 in the right eye with a +0.50 +4.75 X 131, and 20/400 in the left eye with a +1.25 +2.00 X 180. The intraocular pressure was 16 OD and 24 OS by applanation tonometry. There was 2+ nuclear and 1+ cortical cataract formation in each eye.

The right fundus showed 3+ dry age related maculopathy with 4/10 cupping of the optic nerve and a normal periphery. The left fundus showed a cystic appearance to the left macula, 4/10 cupping of the optic nerve, and a negative periphery.

Fluorescein angiography shows an unusual form of cystoid edema in the macular region. His vision has remained the same over a seven-month period with no response to treatment.
