In the right eye I could see the mottled pigmentary change of the retinal pigment epithelium and the papillomacular bundle secondary to the subretinal fluid that she had there a few years ago which must have been somewhat chronic. I can see the ring of subretinal fibrovascular tissue around the disc. This lesion, of course, does not look active presently.

She does have active subretinal neovascularization associated with a ring of subretinal fibrovascular tissue around the disc. It is leaking and creating a sensory retinal detachment of the papillomacular bundle which goes almost up to but not quite into the central fovea. There is also subretinal hemorrhage.

As for etiology, this is probably the same syndrome that we see in young women called recurrent multifocal choroiditis, only it is affecting the tissue around the disc. As for treatment, certainly laser treatment could be done now and I feel certain we can seal down the sensory detachment and get her out of this, but then again this just may do perfectly fine on its own. I wanted to be sure that the vessels were not more severe than they appear to be so we will do a fluorescein angiogram. If this looks good, she will test her vision with Amsler paper and we will just follow this along and hopefully she will get out of this without needing any laser.
