On 9/17/84, a 29 year old man, was struck in the left eye by a metalic fragment while pounding on a pully, part of a machine he had been using to dig a trench in which to lay a water pipe. Visual acuity was RE 20/20, LE 20/30. The right eye was normal. In the left eye there was a 5 mm scleral laceration near the limbus. The anterior chamber was deep with a small hyphema. The lens was clear. The fundus was very hazy, but the disc and posterior pole appeared normal. By X-ray examination a foreign body was noted, moving with ocular movement and apparently in the anterior vitreous.
The foreign body was removed with an electromagnet through the wound of entry and the wound was closed with 5 9-0 nylon sutures. Intravitreal injection of 0.2 ml of saline containing 450 micrograms of clinclamicin and 400 micrograms of gentamicin was carried out through the wound of entry before completing its closure. Postoperatively cefazolin and gentamicin were given parenterally and gentamicin topically. Cultures taken at surgery showed no growth.
On the second postoperative day, when the fundus was more clearly visible, whitening of the retina in the posterior pole was noted. Possible explanations entertained were retinal ischemia and infection. Visual acuity was recorded as 20/200. Fundus photos and a fluorescein angiogram carried out on 9/20/84 are attached, as are follow-up color photos taken on 9/28/84, when visual acuity was 10/200.
