52 year old caucasian emmotropic female and avid golfer was first seen in April 1976, complaining of seeing a circle before the vision of the right eye for ten years. Vision was 20/30, and a subretinal neovascular membrane with hemorrhage and serous detachment of the sensory retina was observed. Argon laser photocoagulation obliterated the membrane and vision was improved to 20/20.
No change was noted for nine years until June 1985, when a giant R.P.E. detachment was noted on a routine annual follow-up exam. She denied visual complaints in spite of alleged daily checks with the Amsler grid. The giant R.P.E. detachment siutated temporal to the fovea had a S.R.N.M. with blocked fluorescence from hemorrhage. Vision was 20/50.
Argon green photocoagulation resulted in resolution of the R.P.E., S.R.N.M., and hemorrhage with recovery of vision to 20/25.
The left eye remains unchanged with rare drusen and a small atrophic area of R.P.E. temporal to the fovea.
This case illustrates the importance of long term follow-up for treatment of macular leaks, the presence of giant R.P.E. detachments and S.R.N.M. in the presence of minimal drusen, and the benefit of photocoagulation.
