58 year old female who noted sudden loss of decreased visual acuity in the right eye six days before examination. No change has been noted since the onset of the problem. The left eye has been completely asymptomatic. She reports having had an ocular examination one and a half years ago at which time indirect ophthalmoscopy was reported to the patient as being normal.

Past medical history is significant for surgery on thyroid (1955) to remove a lump; diagnosis is unknown to patient. She had pneumonia in 1974 and developed peptic ulcer disease in 1977. The diagnosis of rheumatoid arthritis was established by previous examination.

Examination: Corrected acuity OD is 20/400 improving to 20/80 with pinhole and in OS 20/25+-. The patient wears +4 sphere. The anterior segments were unremarkable. The vitreous is clear of cells. Both fundi are entirely normal except for findings in the posterior pole of the right eye.
