This 39 year old Latin American male x-ray technician noticed a total loss of vision in the right eye, except for a small area temporally. Two previous episodes had occurred, one and fourteen years also, in which the vision was lost in a similar manner in the right eye, but returned almost instantly. There is no other significant past history and the patient is in apparent good health. A recent complete evaluation including lab studies was negative.

At the time of examination, the left eye was negative with 20/20 vision. The visual acuity in the right eye was 20/40 with a slight nasal turn. The pupillary reaction OD showed markedly reduced direct light reflex and a 2-3+ Marcus Gunn. The only other positive finding was the appearance of the right fundus.

The visual field in the left eye was full and normal and in the right eye, markedly constricted with a small, irregular nasal field which extended 20 degrees nasally and 10 degrees superiorly and inferiorly.
