A 25 year old white male, who presented with a chief complaint of bilateral decreased vision. He had worked a night shift after which he noticed photophobia upon returning to the day shift. This was followed one week later by a sudden bilateral decrease in central vision (VA was CF at 3 OU). The fundus exam corresponded to the fundus photos. A fluorescein angiogram, ERG, dark adaptation were performed. The ERG showed bilateral decrease in dark adapted and light adapted b-waves and abnormal flicker fusion. The VER was bilaterally abnormal. An examination by both a neurologist and neuro-ophthalmologist were non-conclusive. A C-T scan was interpreted as normal. He has been placed on steroids, but after 6 weeks of follow-up there has been no change in the fundus or in the visual acuity.
