A 52 year old social worker related a history of having blurred vision in each eye for approximately one month. Over the past week prior to the first examination, he developed a large number of floaters in each eye.
Ocular examination revealed a best-corrected visual acuity of 20/70+2 in the right eye and 20/70 in the left eye. The intraocular pressure was 13 mm Hg OD and 15 mm Hg OS. The anterior chamber was quiet in each eye, but in the right eye there were 3+ anterior vitreous cells and in the left eye there were 1-2+ anterior vitreous cells.
Laboratory studies revealed an angiotensin converting enzyme level of 66u/L (nl 8-52). Titers for herpes simplex virus, herpes zoster virus and cytomegalovirus and Lyme disease were negative. An HIV titer was negative. A toxoplasmosis titer was mildly positive. The FTA -ABS was positive and the RPR was also positive at a dilution 1:4. Upon further questioning, the patient revealed that he did have a chancre on his penis approximately one year before.
