58 year old white male who initially presented to an ophthalmologist in November 1997 with decreased vision in his left eye. Visual acuity was 20/20 and 20/30 in the right and left eye. Ophthalmoscopy showed peripapillary retinal pigment epithelial abnormalities of multiple areas of pigment epithelial detachment with an without associated hemorrhage, greater in the left eye than the right. Fluorescein angiographies confirmed the above findings and showed no evide3nce of choroidal neovascularization. A diagnosis of idiopathic polypoidal choroidal vasculopathy was made. The patient was again seen in January 1998 with visual acuity of 20/20 in each eye. Examination was relatevely unchanged. Over the last 3 months and especially in the last two weeks the patient states his vision has become steadily worse. Also noticed was increasing metamorphopsia on amsler grid.
