34 year old black female whose maternal grandmother was a full-blooded Cherokee Indian. She has a history of chronic sinusitis and right otitis media for over 6 months. A CT scan of the sinuses and orbit was performed 3 months prior to presentation and revealed chronic sinusitis with normal globes, orbits, and optic nerves in both eyes. The patient's sinus disease responded well to antibiotics.
On examination the patient's pain was quite severe. She had right facial swelling. 3-mm right proptosis, and 4+ injected conjunctiva in the right eye. Her visual acuity was 20/200 in the right eye and 20/20 in the left eye. Intraocular pressures were 26 mm Hg in the right eye and 14 mm Hg in the left eye. Her right conjunctiva was 4+ injected, and her anterior chamber was narrow. Examination of the left eye was within normal limits.
