On April 6, 1984, a 59 year old woman with a 3 week history of blurred vision presented at the Eye Institute. Her past medical history and family history were negative. Visual acuity in the right eye was 20/20, and in the left eye corrected to 20/20 with a +1.25 +0.50 X 5 degrees. Funduscopy of the right eye revealed drusen in the macula. In the left eye there was a bilobed "notched" serous detachment of the retinal pigment epithelium. Fluorescein angiography revealed prompt staining of the sub-RPE fluid and a prominent "notch," with changes in the adjacent RPE suggesting possible occult choroidal neovascularization at its inferior border (see stereo and angiogram). No treatment was recommended at that time.

The patient returned on May 5, 1984, complaining of further decrease in the vision of the left eye, which was recorded at 20/70. The fundi were unchanged. She returned on May 7 to report a positive scotoma in the left eye. Her acuity was 20/30. She now had a rip in the RPE with folding of the RPE beneath the center of the fovea, as well as serous detachment of the retina (see stereo and angiogram). On May 29, 1984, the vision was 20/30. She was fixating at the site of rolled RPE (see stereo and angiogram). On July 24, the vision was 20/20 and J-l, in spite of the RPE rip and serous detachment of the macula.
